1. Full name, address, and phone number of person and/or company.

2. Detailed description of the images, footage, etc. to be used. (Can be completed after approval.)

3. Detailed description of what the usage is for.

4. Date(s) said images, footage, etc. is to be published, etc.

5. Acceptance that BIGFOOT 4X4, Inc. will be acknowledged (copyrights, trademarks, etc.) in the end product.

6. Formal, written Release (on your companies letterhead) to be reviewed and signed by each party, with your acceptance of the pricing terms.

   • Release may be completed after use of said material has been approved by BIGFOOT 4x4, Inc..

   • Numbers 1 through 5 above should be on the Release.

   • Release must be received prior to delivery of material, either by fax/mail/e-mail.

7. In addition, please provide at least 1 copy of the end product(s) (book, magazine, video of show, etc.) for our records.

Administrative charges or permission fees may be incurred.

Trademark Registration information:

It is unlawful to use any of the above Trademarks for anything other than non-commercial, private purposes. Any other use requires specific permission (a Licensing Agreement) from BIGFOOT 4x4, Inc.  Unlawful use of these Trademarks could result in civil or criminal liability.
